CPI with Reporting
& Annual Assessment Details:
REQUIREMENTS
• ANNUAL INITIAL REPORTING
What: CPI Report Form A, in duplicate
Where: Your resident school district
When: By September 1 or within 30 calendar days of starting instruction under this option
Note: When using this option, you will leave blank the Form A section that asks for a licensed teacher’s name and folder number.
• ANNUAL ASSESSMENT
What: An annual assessment allowed by Iowa law and rule:
- A standardized test approved by the Iowa Department of Education
- A portfolio evaluation by a qualified teacher
- A report from an accredited correspondence school in which the child is enrolled

When: Annual assessment completed by May 31 and the results filed by August 1.
Where: Results are filed at your resident school district
Note: The annual assessment reporting requirement does not start until the child is seven years old (or the first year under this option for older students).

Public school services, activities, classes, or Senior Year Plus options are available.

• Annual assessment reports must include results in 5 specified subjects
Reading, language arts, mathematics, science, and social studies.
SPECIAL EDUCATIONAL NEEDS
• Parents may homeschool students with special educational needs.
While using CPI with Reporting and Annual Assessment, public school and/or AEA services are available through dual enrollment, if desired.

Competent Private Instruction Report
[Form A]
Purpose:
The Competent Private Instruction (CPI) Report [Form A] is published by the Department of Education for use by parents providing CPI with reporting.
The DE's most recent version of CPI Form A is linked below, but it needs several corrections and updates.
We will provide the new version when available.
Outline of Course of Study Form
Purpose:
This form, designed by Homeschool Iowa and reviewed by Home School Legal Defense Association, has been used for decades to comply with the requirement on the CPI Form A for an "outline of the courses of study.
It provides full compliance with the CPI Form A Instructional Program Information item.
Purpose:
This document must be included with your first CPI form filing if you intend to use the exemption for immunizations.
Privacy Form for Dual-Enrolled Student
Purpose:
This optional form is recommended to protect your family's privacy. It informs school officials that you want protections afforded by federal and state laws and that you do not want any information concerning your enrolled child -- including directory information -- to be released to anyone without your prior consent.
Purpose:
This form may be used by a parent when filing the Competent Private Instruction (CPI) Form A.
Ask the school district official receiving your CPI Form to sign the receipt, and retain this receipt with your homeschool records.
Standardized Testing Tools
Purpose:
If you choose to arrange for private – rather than public school or AEA – standardized testing for annual assessment purposes, this form may be attached to the submitted test results to provide added assurance that the standardized test was administered by a qualified test administrator using proper testing protocol prescribed by the test publisher.
Purpose:
Tips and suggestions are provided to prepare your child for standardized testing.
List of Acceptable Tests
Purpose:
This is a list of acceptable tests, that you may use for students under CPI with Reporting & Annual Assessment to satisfy the annual assessment requirement using standardized testing.
Portfolio Evaluation Tools
Purpose:
This optional contract, produced with assistance from Home School Legal Defense Association, is provided to help define and delineate the rights and responsibilities of both parties in a homeschool portfolio evaluation arrangement: the parents and the teacher.
Purpose:
If you will be using the portfolio evaluation method, you can use this simple optional form to fulfill the legal requirement of seeking the school district superintendent's approval of your selected portfolio evaluator. This process is simply a check to see if your evaluator's license is valid and grade-level-appropriate.
Purpose:
This optional form is offered to make the portfolio evaluator's job much easier.
It provides a template tool to assure that all of the required information is included in the portfolio evaluation results report.

Purpose:
Homeschool Iowa provides this non-exhaustive list of correspondence schools for which we have been able to verify accreditation required to qualify as a source of annual assessment reports.
