However, the handbook and form contains errors
and are not current with 2026 law changes.

Iowa homeschool forms for CPI

CPI with No Reporting

As the name implies,
CPI with No Reporting does not require paperwork filing with the public school.

Optional Report Form for CPI Option 2 with Opt-Out (no reporting required)

Purpose:
While all reporting is optional under CPI Option 2 with Opt-Out, parents can file this form to notify their resident school district officials that they are providing private instruction to students of compulsory school attendance age.

Privacy Form for Non-Enrolled Student

Purpose:
This optional form is recommended to protect your family's privacy. It informs school officials that you want protections afforded by federal and state laws, notifying school officials of that information about your child -- including directory information -- is confidential information protected.

CPI with Reporting

Either with Annual Assessment or Supervision
These legal options require the submission of CPI Form A.

Optional Report Form for CPI Option 2 with Opt-Out (no reporting required)

Competent Private Instruction Form A

This is most recent version of the Department of Education's CPI Form A, but it needs several corrections and updates.

We will provide the new version when available.

Outline of Course of Study Form

This optional form, designed by Homeschool Iowa and reviewed by Home School Legal Defense Association, has been used for decades in response to the request on CPI Form A for an "outline of the course of study."

Certificate of Immunization Exemption

This document must be included with your first CPI Form A filing if you intend to use the exemption for immunizations. Find more information here.

Privacy Form for Dual-Enrolled or HSAP-Enrolled Student

This optional form is recommended to protect your family's privacy.

CPI Form A Filing Receipt

This form may be used to create a record of your filing of the CPI Form A for you homeschool records.

CPI Student Eligibility Form

Use for CPI students who are dual enrolled for sports to certify that they meet eligibility requirements set by state associations. The courses listed on this form do not have to be public school courses.

Test Administrator Certification Statement Form

If you use a private standardized testing service for annual assessment purposes, this form may be attached to submitted test results as assurance that the test was administered under testing protocol prescribed by the test publisher.
